Co-Curricular Code Meeting Presentation
If a parent/student was unable to attend a co-curricular meeting offered at Theisen Middle School, they can read this presentation to meet Fond du Lac School Districts Co-curricular code requirements.
All parents and students must read the Fond du Lac School District Co-Curricular Code (or read the presentation linked within the athletics section), sign and return appendix A, B, C, and D of this link before participating in co-curricular activities at Theisen Middle School.
Each student is encouraged to have a physical form filled out by a doctor and on file at Theisen Middle School in order to participate in any activity, club, or sport.  Physicals must be after April 1, of the current year. There only needs to be ONE physical documented for all three years of middle school.
