2013 Flu Mist and Injectable Vaccination Program

This year we will again be offering Flu Mist and Flu Vaccinations in October. Make sure your primary source of insurance is WEA. Dependent children 8 years and older may also attend this clinic. National studies have proven that flu vaccination reduces the health risk and hospitalizations associated with influenza. It would be helpful if you print and complete the attached consent form prior to attending the clinic.

Flu Mist is indicated for healthy children, adolescents and adults 2 to 49 years of age. If you are NOT enrolled in WEA Trust, you may still participate in this program but will be responsible for the $25 cost of the vaccination.

The Fond du Lac County Health Department will again be our provider for the Flu Vaccination Program.
