Co-Curricular Code Meeting Presentation
If a parent/student was unable to attend a co-curricular meeting offered at Woodworth Middle School, they can read this presentation to meet Fond du Lac School Districts Co-curricular code requirements.

Fond du Lac High School/Middle School Co-Curricular Code

All parents and students must read the Fond du Lac School District Co-Curricular Code (or read the presentation linked within the athletics section), sign and return appendix B & C of this link before participating in co-curricular activities at Woodworth Middle School.

Middle School Physical Form

Each student must have a physical form filled out by a doctor and on file at Woodworth Middle School in order to participate in any activity, club or sport. 6th grade physicals must be after 4/1/19. There only needs to be ONE physical documented for all three years of middle school
