This medication authorization from a prescribing physician is required to be completed and presented to the school a child attends before any prescription drug may be administered to the child.
This is the form that parents take to the doctor's office for incoming kindergarteners to be completed by the physician, physician assistant, or nurse clinician.
This is the form that parents take to the doctor's office for middle school students to be completed by the physician, physician assistant, or nurse clinician.
Please print and complete only one of these copies for submission to your school.

Please complete these documents if you are interested in volunteering in our schools. This would also include any parents who have been asked to chaperone a field trip. Return the forms to your school office or to the Administration Center, 72 W. 9th Street, School Health and Safety program.
